In traditional open surgery, an incision is made in the skin and the abdominal wall (muscles) to expose the area where the surgery will be performed. The exact site and shape of the incision will depend on the exact operation being performed. The main advantage of open surgery is in the exposure and access it provides to the relevant internal structures. Given its advantages, open surgery is usually preferred for the larger or more complex cancer surgery.
